Permethrin cream contains Permethrin, a pyrethroid compound used as a topical scabicidal agent for treating infestations caused by Sarcoptes scabiei (scabies). It disrupts the sodium channel function on the nerve cell membrane of the mites, causing delayed repolarization, which results in paralysis and the eventual death of the pests.
Permethrin is applied topically, and there is limited data on its systemic absorption. A small fraction (less than 0.5%) of the cream is absorbed through the skin within the first 48 hours. Any absorbed permethrin undergoes rapid metabolism via ester hydrolysis, likely in the liver, and its byproducts are excreted primarily through the urine.

- Adults and children over 12 years: Apply a full tube.
- Children aged 6-12 years: Use up to half a tube.
- Children aged 1-5 years: Apply up to one-quarter of a tube.

- For children under 2 months, the appropriate dose is not established.

Before applying permethrin cream, ensure the skin is clean, cool, and dry. Avoid hot showers or baths just before use.
For adults and children over 2 years: The cream should be applied to the entire body from the neck down. It should be gently rubbed into the skin until fully absorbed, covering all areas, including between fingers and toes, under the nails, and on the soles of the feet.
For babies under 2 years: Apply to the face, neck, ears, and scalp, avoiding the areas around the eyes and mouth.
Leave the cream on for at least 8 hours before washing it off. If any area of the skin is washed during this time (e.g., after handwashing), reapply the cream to that area. If required, the treatment can be repeated after 7 days based on a doctor’s advice.
